UPDATED: 10/14/20 Federal Impact Aid Survey Form

Mahalo to those who have completed and turned in your Federal Aid Survey Card back in September. However, due to the ongoing changes, the Federal Government has decided to issue a new survey card (dated 10/14/20). These surveys were in the packets that were picked up from school on October 22-23.

Once, again, we need your help in completing one survey, per child attending Ha‘aheo Elementary. Based on the information gathered from these surveys, our school can receive extra funding.

Please send your completed surveys back to our office in the self-addressed, stamped envelope that has been provided.

Mahalo for your time and attention to this matter!

The overall goal of this survey is to better understand how our Ha‘aheo families are feeling during this pandemic about school-home communication, implementation of distance learning, ‘ohana well-being and need for support.
